These are the two islands I pair with Santorini most often, and they answer different trips: Paros if you plan around dinner, Milos if you want to be in the water every day. Both keep small Athens-served airports — and both are the reason the middle of a route beats its bookends.

Choose Paros
If the table sets the day
Mykonos’s good looks, better food, a third of the attitude. Naousa on foot at night, a sailing day to Despotiko’s empty beaches, and hotels — Parīlio, Cosme — that photograph well and still work when you’re actually living in them.
Choose Milos
If the water sets the day
Volcanic rock and impossible water — Milos looks like nowhere else in Greece, and it’s best seen from a boat. Kleftiko’s sculpted white cliffs are the reason you came; Sarakiniko’s moonscape is the encore.

That’s genuinely the whole decision, and it’s why these two are my B and C pairings with Santorini rather than rivals — they solve different trips.
If you can’t choose, the tell is the boat: on Milos the boat day isn’t an excursion, it’s the island — you experience Milos from the water or you miss it. If that sentence excites you, book Milos. If you’d rather be walking to dinner in Naousa while someone else worries about the wind, that’s Paros.
